Request a Copy of Your Form 1095-C

In accordance with the Paperwork Burden Reduction Act (PBRA), Windigo Logistics uses the Alternative Furnishing Method (AFM) for ACA reporting. This means Form 1095-C is not automatically mailed to employees. Instead, any employee or other responsible individual may request a copy at any time.

How to Request Your Form 1095-C

Responsible individuals may request a copy of their Form 1095-C using any of the following methods:

Local HR Office: Requests can be made in person at your local HR office.

Important State Information

If you reside in a state that mandates employers to furnish a 1095-C separately, you may continue to receive additional documentation as required by state law.
